Molluscum contagiosum: when to observe and when to discuss treatment
Molluscum contagiosum is a viral skin infection causing small raised bumps, often with a central dimple. Many uncomplicated cases in otherwise healthy people resolve naturally, but the time varies. Treatment is a decision to make around symptoms, site and the burden of the condition.
Observation can be reasonable
A systematic review and network meta-analysis compared several treatments and also recognised observation for asymptomatic infection. Adverse-effect data were limited. A ranking from mixed trials should not become a rule that every child needs the same intervention.
The CDC clinical overview notes spontaneous resolution and the potential for some physical treatments to cause pain or scarring. Ask what treatment would add in the actual situation.
What makes assessment more useful?
Seek advice for an uncertain diagnosis, troublesome surrounding eczema, lesions near the eye or in the genital area, or impaired immunity. Increasing pain, spreading redness or fever warrants medical advice. Redness in a bump can have more than one cause and should not automatically lead to antibiotics without assessment.
